Patient Forms

1. Does my insurance plan cover all my costs?

We make every effort to keep down the cost of your health care at Longfellow Holistic Health Center. You can help by calling for our current fee schedule and by paying for treatment at the time services are rendered. We accept credit cards, cash, or personal check. Our providers are in‑network with all major insurance companies. Please call our office manager to verify coverage with your insurance or discuss other payment plan options.

2. How long are the PT appointments?

Your first visit will last approximately one hour and will include a comprehensive evaluation and applicable treatment administered by a licensed physical therapist. Your physical therapist will review your medical history, diagnostic tests and any recent events that may have contributed to your current condition. Your evaluation will include assessment of your current functional deficits, pain level and posture, as well as a thorough evaluation of your flexibility, strength, balance and endurance. Following the first appointment the subsequent treatment sessions average between 30 – 45 minutes.

3. What is the preferred attire for a PT appointment?

Generally lose fitting, work out clothing preferred with easy access to Upper and lower extremities.

4. What is our cancellation policy?

We have a 24 hour cancellation policy as it provides more availability and appointment times for our patients and maximizes our practitioners’ schedule.

5. What is the difference between a prescription and a referral, and do I need a prescription for PT services?

A prescription is a doctor written recommendation for you to receive Physical Therapy. A prescription must include your name, diagnosis, doctor’s signature, and date and recommendations for treatment options. You must bring your prescription to your initial visit.

A referral is an authorization that is submitted to your insurance company usually by your primary care physician (PCP). Referrals require the submission of the Practitioners National Provider Identifier (NPI) number to the PCP.

In Massachusetts direct access to physical therapy is allowed which means a referral or prescription are not required to have physical therapy.  However, most insurances require a referral/prescription for reimbursement of physical therapy services. If you choose to seek PT services without insurance, you can request a quote for our rates.

6. How frequently do I need to go to physical therapy?

Generally you will be seen for 2-3 visits per week depending on your diagnosis and the treatment plan that has been outlined by your PT.
